Property Owner Name(s): please list the legal owner(s) of the property in question.

Address of Proposed Improvement: the house number and street is sufficient. The legal property description is not required for the BPNA although it may be required by the city for any necessary permits or your bank for any necessary loans.

Please Describe planned work - attach drawings, photos, etc.: The information shown here must allow a reasonable person to understand the changes you are planning and the appearance of the exterior of your house following the changes. Some appropriate items should include:

• a brief summary of the total project (required)
• description of materials to be used. Any materials that differ from current home construction should be clearly specified (for example: BPNA homes are brick and wood siding. Adding a metal storage building would be a different construction material. Designing or painting the storage building to match the house can reassure your neighbors that the building will be attractive and "fit" the neighborhood. Include any information that may be important to your neighbors.
• drawings with dimensions
• photos or brochures of manufactured materials (ex: awnings, roofing, windows, etc.)
• reference to any similar construction within Bryan Place (ex: brick mailboxes, etc.)
• whether the changes are visible from the front side, back or all directions. Alternatively, specify if a privacy fence prevents view by neighbors.
• any other materials or information to clearly explain your project to your neighbors. to the Architecture Chair and to the BPNA Board. The more information provided, the easier it is to make a decision in a short period of time.

Affected Property Owners: contact the neighbors who will be able to view the improvements you propose; provide their names, addresses and secure their signatures with dates. These neighbors must indicate whether they have objections to the proposed changes. Room is allowed for six (6) nearby residents. If more room is necessary, attach additional sheets. All neighbors who will be able to see the changes from their homes, yards, drives, etc., should be contacted.

Bryan Place Neighborhood Association Board Review: This area is completed by the Board. Upon review, the Board's decision will be sent to you as a copy of this form. If approved, you will be provided a letter stating that the Board has no objections to your project if completed as described and in compliance with all building codes, permit requirements, zoning restrictions and city ordinances. You should retain a copy of the completed form and letter with your important home ownership papers (mortgage, survey, remodeling contracts, etc.).
